Documents required for admission to the JOVIMED Senior Home

  1. Application for admission to the Senior House JOVIMED -  Download Print

  2. Current health certificate or information card from hospital treatment (if the hospital discharge has taken place in the last 30 days) - Download Print

  3. Photocopy of the information card from the last hospital treatment

  4. List of medications accepted by the person applying for admission to the JOVIMED Center - Download Print

  5. List of persons responsible for admission / legal representatives - Download Print

  6. Identity card or other document confirming the identity of the person applying for admission to JOVIMED

Completed documents can be delivered to JOVIMED as follows:

Patients admitted to stay at the JOVIMED Center should have the following personal items:

products for oral hygiene (toothbrush, toothpaste, denture, denture cleaner)

shaving accessories

towels

shampoo

utensils and products for the care of hands and nails

​footwear (good adhesion to the ground), flip-flops

pajamas, bathrobe

sports tracksuit

Jovimed is a care provider, and should not be regarded as cure provider. Consequently, residents and/or family members of the residence should accept that Jovimed has committed to employing external medical staff regularly, but unlike hospitals, it can not guarantee 24/7 medical services
